Evaluation Activity: Quantitative and Qualitative (Sample) Evaluation Sheet


Note:

This a sample evaluation sheet with both quantitative and qualitative information. In the first two questions (or more, depending on the number of objectives) participants are asked to assess the extent to which the workshop objectives have been met. To answer they must select one of the numbers for each question, ranging from 1 (the objective hasn't been met at all) to 5 (very much). The following questions will provide you with qualitative information.
